Nomination Form Instructions

In order to fill out the nomination form you will need the information listed below:

Nominee's details
Title, First Name, Middle Name, Last Name Position of Nominee Work Phone Number, Home or Mobile Phone Number Fax, Email Postal Address of Nominee Name of Nominee's Institution of Affiliation and Department Postal Address of Institution of Affiliation Summary of Nominee’s achievements meriting the Prize

Attachments
A detailed description of the nominee’s work (1-2 pages), stipulating reasons why the nominee merits the Dan David Prize. Please refer to the importance of the work, its impact, and its projected influence in the relevant field (allowed: doc\|rtf\|docx\|pdf / max:5mb) A full Curriculum Vitae of the nominee, or a detailed prospectus in case of an organization nominee (allowed: doc\|rtf\|docx\|pdf / max:5mb) A list of the nominee’s publications of the last 15 years (allowed: doc\|rtf\|docx\|pdf / max:5mb) A list of the nominee’s most relevant publications or projects (allowed: doc\|rtf\|docx\|pdf / max:5mb) Others A photo of the nominee in electronic format, to be used by the Dan David Prize in case of need (300 dpi, copyright free, high-definition - allowed: jpg or tif / max:5mb)

Joint Nominations
Please fill in two separate forms, attaching the relevant documents for each nominee. The 'Last Name' and ‘First Name’ fields should be filled in as follows: Form 1: In the 'Last Name' field, type 'Joint Nomination Name A/Name B – Name A’ (and all the details and attachments pertaining to A). In ‘First Name’ field type the first name of A. Form 2: In the 'Last Name' field, type 'Joint Nomination Name A/Name B – Name B’ (and all the details and attachments pertaining to B). In ‘First Name’ field type the first name of B.

Joint Nominations

Please fill in two separate forms, attaching the relevant documents for each nominee.
The 'Last Name' and ‘First Name’ fields should be filled in as follows:
Form 1: In the 'Last Name' field, type 'Joint Nomination Name A/Name B – Name A’ (and all the details and attachments pertaining to A).
In ‘First Name’ field type the first name of A.
Form 2: In the 'Last Name' field, type 'Joint Nomination Name A/Name B – Name B’ (and all the details and attachments pertaining to B).
In ‘First Name’ field type the first name of B.

Confirmation of successful submission will be provided.
A copy of the nomination material will be sent to the nominator by email.
